Salary:        £25,000 pro rata depending on experience 

Hours:         25 hours a week (over 3 days)

Location:    Home based with travel across Hampshire 

                      (You must hold a valid driving licence and have access to a vehicle. The vehicle must be                            insured for Business use).


Citizens Advice Hampshire are recruiting a Home & Well Project Coordinator for their expanding Home & Well project.  We are seeking a creative, confident, enthusiastic and proactive Coordinator to join our skilled team of Advisers.

Home & Well is a unique collaboration by organisations working together to support residents of Hampshire and the Isle of Wight to return home as quickly and as easily as possible after a hospital stay and, importantly, ensure they have the comfortable home environment that will help them continue to recover rather than relapsing.

The Project Coordinator will focus upon initial training, monitoring and supporting the Advisers who deliver the help and support to those vulnerable people who are being discharged from hospital.
